WELLNESS PROGRAM

TruckPro has partnered with HealthyGuidance to provide an opportunity for you and your spouse enrolled in TruckPro’s medical
plans to take charge of your health. This free, confidential program will help you and your family make positive lifestyle changes with
expert support, resources and information.
Reimbursements of up to $500 per year for associates and up to $500 for spouses ($1,000 total rewards for you and your spouse)
will be awarded for participation in the wellness activities listed below. Your rewards will be deposited in your HSA account if you are
enrolled in the Platinum or Bronze plan or paid in cash if enrolled in the Silver plan.
You can earn $200 for completing your Annual Physical Exam and no paperwork is required.
