        LIVE-WELL WELLNESS PROGRAM OVERVIEW


        Our mission at Fort HealthCare (FHC) is to “improve the health and well-being of our community”, and as such, we are as
        committed to caring for our staff as you are caring for others in our community.

        We at Fort HealthCare have received reduced healthcare contribution rates - largely due to our applying healthy behaviors
        in our lives.  Wellness is an active process for each of us, becoming aware of, and making choices towards a healthy and
        fulfilling life.  It is not about being totally free of illness - rather it is about the process of change in our behaviors and
        habits towards making those healthy decisions for the benefit of our own health.

        Our Wellness Program is both outcomes-based and participation-based.  Personalized advisor visits may be required for
        some of us – based on each of our outcomes.  These Advisor visits support and encourage healthy decisions, help in
        understanding your outcomes (numbers), and education to help guide each of us in achieving our health goals.  Although
        there are required components to this program, you have the option to personalize how you earn points – basing activities
        on our own wellness interests and health needs.
        The Fort HealthCare Wellness Program offers a great opportunity for you to be “in-tune” with your health, and you have
        many different opportunities for maintaining/improving your numbers, learning about nutrition, physical activity and
        other wellness topics, and to earn health reimbursement arrangement (HRA) or flexible spending account (FSA) dollars
        through the outcomes-based component AND monetary incentives through the participation-based component.

        What are the outcomes-based and participation-based components in the program?

            •  Outcomes-based
               o  Biometric screenings provide insights as a baseline assessment of your health status and potential medical
                   risk factors for chronic preventable diseases such as diabetes, heart disease, and hypertension.  From these
                   screenings, strategies for improving your health and well-being can be identified (if necessary) to assist you in
                   achieving your healthy goals.  Incentives paid into HRA (see page 6) or FSA accounts .

            •  Participation-based
               o  Earn points for wellness activities completed throughout the year.
               o  You have the potential to earn extra monetary payouts provided by Fort HealthCare based on points
                   attainment by mid-year (July) and again by program year-end (January).

                                                  If you are unable to participate in any of the health-related activities or
                                                  achieve any of the health outcomes required to earn incentive, you may be
                                                  entitled to a reasonable accommodation or an alternative standard.  You
