Diabetes Care Rewards Program

The Good Health Gateway® Diabetes Care Rewards Program helps individuals with diabetes and
pre-diabetes lead healthier lives through effective management of their condition. Employees
and dependents enrolled in a Plymouth Rock medical plan will receive $0 co-pays on related
medications and supplies by completing the program steps.

Reasons to Join
• $0 Co-pays. You pay $0 for related medications and supplies when you meet the requirements of the program.
• Get Support & Reminders. You will receive reminders through mail, email and text, so once you earn your
rewards, you keep them!
• Private & Confidential. Your participation is free, voluntary, and confidential. Your personal health care
information is not shared with Plymouth Rock.
PHYSICAL WELL-BEING Getting Started

Step 1: Register for the program on GoodHealthGateway.com or by calling the Good Health Gateway® confidential
Help Line at 800-643-8028.
Step 2: Complete a brief phone call with a Diabetes Nurse Educator to get your written Diabetes Health Action Plan®
Care Guide to review and share with your doctor. Call 800-643-8028 to schedule your meeting.
Step 3: Submit a form to verify completion of required medical exams (including virtual) and lab tests including annual
eye and foot exams, fasting blood lipid level, urine/protein level and Hemoglobin A1c test.
