Life and AD&D Insurance – The Hartford

        Life  insurance  benefits  can  provide  your  family  with  peace  of  mind.  Murata  provides  you  with  basic  life  insurance
        benefits and basic accidental death and dismemberment (AD&D) insurance, each equal to your annual pay, up to a
        $500,000 maximum. AD&D coverage provides a benefit in the event of accidental death or a serious injury. Murata pays
        the full cost of basic life insurance and basic AD&D coverage.




                     Disability Insurance – The Hartford
       If you should require an extended absence from work due to an illness or injury, Murata provides you with access to
       disability benefits, so that you can focus on your recovery. Peace of mind during an illness or injury can assist in the
       healing process.

       Murata’s short-term disability (STD) plan can offer you up to 100% of your pay for up to 12 Weeks. Murata pays the full
       cost of your STD coverage.  STD benefits are based on your years of service with the company. If you have:
                  •   Fewer than five years of service, you receive 60% of your pay for up to 12 Weeks.
                  •   Five years to less than 10 years of service, you receive 100% of your pay for the first 6 weeks and 60% of
                     your pay for the remaining 6 weeks.
                  •   10 years or more of service, you receive 100% of your pay for 12 Weeks.
       If your illness extends beyond 12 Weeks, long-term disability (LTD) coverage will pay 60% of your pay, up to $10,000 a
       month. Murata pays the full cost of LTD coverage.




        Employee Assistance Program (EAP) – Guidance Resources:

        ComPsych Program


        Murata  recognizes  that  problems  of  a  personal  nature  can  have  an  adverse  effect  on  employees  and  their  job
        performances  and  that  most  problems  can  be  dealt  with  successfully  when  referred  to  the  appropriate  professional
        resources.

        The  Employee  Assistance  Program  (EAP),  administered  through  Guidance  Resources,  is  a  confidential  program
        designed to deal with a broad range of human problems such as mental or  emotional disturbance, family or marital
        difficulties, alcoholism or other drug abuse, and financial stress or other concerns.

        Guidance Resources with The Hartford will provide 3 (three) face-to-face meetings annually, identification of resources,
        and referrals to help promote emotional wellness. The EAP program is available to all active employees. You do not need
        to be enrolled in a medical plan to utilize this program.
