Benefits for You and Your Family
The Goodwill Columbus (referred to as “Goodwill” in this document) Benefits Program is designed to provide
you with benefit solutions that help you stay healthy, feel secure, and help you maintain life balance.
As a Goodwill employee, you have the opportunity to participate in a variety of insurance plans including:
• Medical • Short Term Disability – Basic and Buy-up Option
• Dental • Long Term Disability
• Vision • Supplemental Term Life and AD&D
• Basic Life and AD&D • Critical Illness
• Dependent Life • Accident
We encourage you to review all your options before making your benefit elections. Only you can determine
which benefits are best for you and your family. We want you to understand all your options and make
informed decisions. Once you elect your Goodwill benefits, your elections remain in effect for the current plan
year through December 31, 2022.
Goodwill offers a comprehensive suite of benefits to promote health and financial security for you and your
family. This booklet provides you with a summary of your benefits. Please review it carefully so you can
choose the coverage that’s right for you.
Employee Eligibility –
All full-time Goodwill employees are eligible for Health and Welfare benefits on the first of the month
following 30 continuous days of employment or transfer to full-time status. Benefit elections are made within
30 days.
Hours Worked Requirement
Employees who work 30+ hours per week are eligible for all benefits herein.
All employees are eligible for Health Advocate, retirement, and employee assistance programs,
with no minimum hours requirement.
Dependent Eligibility –
You may enroll your eligible dependents for coverage once you are eligible. Your eligible dependents include:

Dependents:
- Medical, Dental, Vision: Available for dependent children until the end of the month in which
they turn 28

- Orthodontia: Treatment must begin before 19 birthday
- Optional Life: Available for dependent children age 15 days to 26 years
