        ENROLLMENT & CHANGES


        When and How Do I Enroll?

        New hire eligibility begins the first day of the month following thirty (30) continuous days of employment.

        The Goodwill HR Service Team is here to assist with answering your questions when you cannot find the
        answers you are seeking. Your benefits enrollment will be completed online as part of your Dayforce
        employee self-service portal. Please log on to Dayforce and click the “Benefits” tab to locate any

        enrollment information currently available to you.

        Changing Coverage During the Year

        Changes can be made to your benefits during the year only when you experience a qualified change in
        status, also known as a Qualified Life Event.  You must notify the Human Resources Department within

        30 days of a qualified change by completing the online “Life Event Declaration” form in your Dayforce
        employee self-service portal. This form must be completed within 30 days of the event, and the election
        must be consistent with the event. For example, if your dependent child no longer meets eligibility

        requirements, you can drop coverage only for that dependent. Depending on the type of event, you may
        be asked to provide proof of the event. If you do not contact Human Resources within 30 days of the
        qualified event, you will have to wait until the next annual enrollment period to make changes (unless
        you experience another qualified life event).


        Qualified Life Events:
          Marriage

          Divorce or legal separation
          Birth of your child
          Death of your spouse or dependent child
          Adoption of or placement for adoption of your child
          Change in employment status of employee, spouse or dependent child
          Qualification by the Plan Administrator of a child support order for medical coverage

          Entitlement to Medicare or Medicaid
