Cigna OAP In-Network Medical Plan

The Cigna OAP In Network (OAPIN) plan provides access to their nationwide network of Cigna providers with
no referrals required to see network providers. The OAPIN plan has a $15 office visit co-pay. Preventive services,
as defined under health care reform, are covered at no cost. The plan provides in-network office visits, simple
lab/x-rays and prescription drugs at a co-pay. All other services, including the Emergency Room, are subject to
the $250 deductible, after which you pay 10% of negotiated charges. Employees and their families should verify
all providers you are referred to are in fact in the Cigna Open Access Plus In network. There is no out-of-network
coverage on this plan.
Utah Employees: This plan does not include Intermountain Health (IHC) Providers.

This Plan is closed to new enrollments. You may only select this plan during Open Enrollment if you are currently
enrolled. This plan will be no longer be an option on January 1, 2023.
