Cigna OAP/PPO Medical Plan
The Cigna OAP/PPO plan has a $20 co-pay for in-network office visits. Preventive services, as defined under health
care reform, are covered at no cost. The plan provides in-network office visits, simple lab/x-rays and prescription
drugs at a co-pay, not subject to the deductible. All other services, including the Emergency Room, are subject to the
$500 deductible, after which you pay 20% of negotiated charges for in-network services. Out-of-network services
will result in significantly higher costs. Employees and their families should verify all providers you are referred to are,
in fact, in the Cigna Open Access Plus network.
Utah Employees: This plan includes most Intermountain Healthcare (IHC) network providers and facilities.
