The ChatWellMD™ mobile application, powered by Innovaccer Inconnect, connects Members to CCPHP' s
concierge physician.
Description & Sample Language:
In order to provide better connectivity with ' your chosen' physician and the
Membership Program, Members are provided with CCPHP’s custom digital and mobile media application, ChatWell MD™.
This HIPAA compliant platform can be used from mobile devices, other hand-held devices, or computers to directly
contact ' your chosen' physician, or attend Telehealth visits (regardless of whether such visits are medically
necessary).

Through the ChatWell MD™ App you can:
● Communicate directly with your doctor using chat (text) functionality
● Send pictures & files, if required
● Be invited to a telemedicine (video) consult with your doctor
● Request telemedicine (video) consultations with your doctor
