Ms. Phyllis Stanley
Columbia, SC
On October 17, 2019, the AMT Office received information from Ms. Kayla Spires, Clinical
Practice Coordinator of Quality & Clinical Support from Prisma Health. Ms. Spires reported that while
employed by Prisma Health, Stanley had been identified as calling in a prescription for a controlled
substance for herself. An investigation was initiated by the pharmacy that received the
prescription. The clinical practice confirmed the findings following the investigation. Stanley admitted
calling in the prescription and she was terminated from employment with Prisma Health. The
revocation letter was sent on October 22, 2019 giving Stanley 30 days to respond. No response was
received within 30 days. A final revocation letter was sent on November 25, 2019.
Mr. Marquise Perry
Kershaw, SC 29067
The AMT staff received a report on October 9, 2019, that while working as a pharmacy technician
at a CVS store in Rock Hill, SC, Perry made photographic images of one or more customers’ credit cards
and used the information on those images to make a number of fraudulent transactions. One of the
fraudulent transactions that Perry engaged in was an online renewal of his AMT membership dues using
the credit card number of a CVS customer with whom Perry had recently conducted a transaction at the
pharmacy. The Rock Hill Police Department obtained video recordings of his activities while on duty at
the CVS and confirmed that Perry had taken photos of customer credit/debit cards on his phone.
AMT subsequently learned that Perry engaged in additional instances of credit card theft and
fraudulent transactions in the Charlotte, NC area. In particular, on the morning of December 13, 2019,
during a visit to his former place of employment (Charlotte Medical Clinic), Perry stole a credit card from
the unattended purse of a former coworker and shortly thereafter made at least two unauthorized
purchases using the card at Harris Teeter grocery stores. The Charlotte-Mecklenburg Police Department
provided a report detailing these allegations and stating that Perry had been arrested on January 2 of
this year. The police report further indicated that Perry had admitted to the crimes at the time of his
arrest.
The proposed revocation letter was sent on January 10, 2020. The member responded within 30 days to
request leniency. He stated that he was depressed over the death of his mother. The Judiciary
Committee reviewed his request but denied leniency due to the seriousness of his offenses. A final
sanction letter was emailed to Perry on January 28, 2020. Perry was advised that he could appeal the
decision or petition the board to reinstate in two (2) years. No appeal was submitted within the allowed
30-day period.
