AMT staff members were able to determine that the membership card Elder presented to Emory
               Healthcare was not authentic and instead had been falsified.  AMT staff also determined that no one by
               her name or Social Security number had ever been certified through AMT or had taken a certification
               examination offered by AMT.  Staff determined that Elder had created an online profile with AMT in
               2003 and had ordered a practice exam at that time; however, she never proceeded to take or pass the
               RMA(AMT) certification exam, or any other exam administered by AMT.

                           The cease and desist letter was sent on July 9, 2019.  In addition, the use of fraudulent
               documentation in association with any aspect of the certification process results in indefinite
               disqualification from future certification with AMT.  Accordingly, she would not be eligible to apply to
               take an AMT-administered certification test until July 9, 2021, at which time she would need to meet
               current eligibility standards for the applicable examination and would need to petition the AMT Board of
               Directors for permission to take the exam.  No such petition will be accepted or considered prior to the
               expiration of the two-year period ending 7/9/2021.

               3.  Ms. Vivian Oquendo Matta
                    North Richland Hills, TX 76180

                           Ms. Matta attempted to obtain or maintain employment using a forged verification letter,
               purportedly issued by AMT, indicating that she was currently certified as a Registered Medical Assistant
               (RMA) with a certification expiration date of November 1, 2020.  The forged letter was submitted for
               AMT’s review by the Nurse Manager for the Northeast Division of Family Healthcare Associates.

                       The letter obviously had been falsified by Matta.  A review of AMT records discloses that she
               applied for RMA certification and took the RMA exam in November 2015 but did not achieve a passing
               score.  Her application expired in October 2017 without her having registered to re-take the exam.  No
               one with her name or social security number has ever been certified by AMT.  Legal counsel sent a
               cease-and-desist letter via email on 9/10/2019.  Ms. Matta immediately responded denying that she
               falsified the letter and stated that she received the letter in question from AMT (an assertion that was
               demonstrably false).  However, she stated that she would abide the cease and desist order.

               4.  Ms. Marie Castillo
                    Louisiana

                       On September 9, 2019, the AMT Office was informed that Castillo had attempted to obtain
               employment as a medical assistant using a falsified examination score report which purported to
               indicate that Castillo had passed the Registered Medical Assistant (RMA) exam administered by
               AMT.  When Castillo was asked for verification of her RMA credential, Castillo presented a fraudulently
               altered exam score report to Melanie Nash, VP of Operations and Human Resources for Any Lab Test
               Now in Lafayette, Louisiana.  Ms. Nash asked the AMT Office staff to verify whether the score report was
               authentic, and the staff immediately was able to determine that the letter had been falsified.

                       The exam score report Castillo presented to Ms. Nash does not belong to Castillo, but rather,
               was issued to a current AMT member whose ID number appears on the report upon her passing the
               RMA exam.  Castillo evidently deleted her name from the report (all such score reports include the
               examinee’s name), leaving a blank space where the name should appear.  In a follow-up email
               communication with Ms. Nash, Castillo admitted that, in her own words, “I've been working for long
               time in the hospital and I'm not [a] medical assistant….”
