It is unknown how Castillo came into possession of an exam score report issued to a legitimate
AMT member, but in any event, her alteration and falsification of an AMT examination score report
constitutes theft of AMT’s intellectual property., Further, Castillo is falsely holding herself out to
employers and the public as an AMT-certified Registered Medical Assistant constitutes deceptive trade
practices and/or health care fraud under state laws, and could jeopardize the health and welfare of
patients under the care of any employer who may hire Castillo as a certified professional based on the
false credential.
Castillo was directed to immediately cease and desist from continuing to represent herself as
certified by AMT or otherwise make any use of the forged score report on September 10, 2019. In
addition, the use of fraudulent documentation in association with any aspect of the certification process
results in indefinite disqualification from future certification with AMT. Castillo was informed that she
is not eligible for certification by AMT in any discipline that it certifies.
Castillo responded immediately by email stating that she was sorry and would destroy all copies
of the forged documents.
5. Ms. Latrice Faulkner
Memphis, TN 38125
Ms. Faulkner attempted to obtain or maintain employment using a falsified image of a
membership card purportedly issued by AMT, indicating that she was currently certified by AMT as a
Registered Medical Assistant (RMA) with a certification expiration date of August 3, 2020. A copy of the
forged card was submitted for AMT’s review by a supervisor at Christ Community Health Services in
Memphis, where Faulkner had been employed.
The card image obviously had been falsified by Faulkner. A review of AMT records discloses that
she created an online profile on AMT’s website on May 30, 2019, but she did not take or pass the RMA
certification exam. She never became certified as an RMA or in any other category for which AMT offers
certification. Additionally, the AMT ID number on the card she presented belongs to a different
individual who is a resident of another state and is no longer actively certified through AMT.
Faulkner was directed to immediately cease and desist from continuing to represent herself as
certified by AMT or otherwise make any use of the forged card or card image on October 16, 2019. In
addition, the use of fraudulent documentation in association with any aspect of the certification process
results in indefinite disqualification from future certification with AMT. Faulkner was informed that she
is not eligible for certification by AMT in any discipline that it certifies. She responded by telephone on
October 17, 2019 indicating that she would comply with the order and also she would send an email
stating the same.
6. Ms. Randi Duncan
Yukon, OK 73099
Duncan attempted to obtain or maintain employment as a medical assistant at Digestive
Disease Specialists in Oklahoma City using a falsified document purportedly issued by AMT, indicating
that she was currently certified as a Registered Medical Assistant (RMA) with a certification expiration
date of 2/1/2022. The document was submitted for review by the employer.
