A comprehensive patient interview would include the following components of the patient's health

Chief complaint (CC)
History of present illness (HPI)
Past medical history (PMH)
Health maintenance and immunizations
Family history (FH)
Personal and social history (SH)
Review of systems (ROS) and physical examination (PE)

Communication skills are essential in obtaining effective subjective information.
Conducting an efficient patient interview is a skill that develops with practice over time and
involves asking appropriate questions and actively listening to the patient.
Communication skills are the foundation of many parts of the PPCP
2- Assess
"The pharmacist assesses the information collected and analyzes the clinical effects of the
patient's therapy in the context of the patient's overall health goals in order to identify and
prioritize problems and achieve optimal care."
(1) medication assessment(appropriateness, effectiveness, safety, and adherence)
(2) patient history and risk assessment (health and functional status, risk factors, health data,
cultural factors, health literacy, and access to medications ..)
(3) preventive care assessment.(if a patient presents during influenza season, the pharmacist
should assess the patient's influenza immunization status.)
