Employment Related Injuries

NWF provides a comprehensive workers compensation insurance program at no cost to you.

This program provides a method for reporting accidents, a system of referral of injured
employees for medical treatment of any injury or illness sustained in the course of
employment, and maintenance of an accident prevention program.

Subject to applicable legal requirements, workers compensation insurance provides for the

payment of medical expenses incurred if you are injured, compensation if you are unable to
work, or compensation to your dependents in the event of your death due to an accidental
injury or occupational disease arising out of and in the course of your employment with NWF.
It is administered by the insurance company which is responsible, in coordination with the
appropriate state commission office, to manage the program including the determination of
                                      l
                                     c
        benefit eligibility of each claim.
Workers compensation benefits may not be extended to you when it is established that your
injury or death was due to willful misconduct, intentional self-inflicted injury, the result of an
attempt to injure another, or caused by intoxication/substance abuse. Likewise, any injuries
resulting from the willful failure or refusal to use a prescribed safety device or perform a duty

required by law, NWF rule, or regulation are not covered.

If you sustain a work-related injury or illnesses you must inform your supervisor immediately.
No matter how minor an on-the-job injury may appear, it is important that it be reported
immediately. If you need medical treatment for a work-related injury or illness you must
contact Human Resources at 703-438-6494 for coordination of necessary care. In addition, if

you are injured or involved in any incident with potential injury, an Employee Accident
Investigation Report must be completed and forwarded to Human Resources as soon as
        possible.

Neither NWF nor the insurance carrier will be liable for the payment of workers compensation
benefits for injury that occurs during your voluntary participation in any off-duty recreational,
social, athletic activity or non-NWF events occurring on NWF property.
