Single Disability includes relapses of the original disability occurring within a 12 month period












        even if the absences are separated by periods of active employment.  Base Compensation is











        defined as regular pay at the time  disability  leave begins, excluding overtime or other

        additional compensation.
        Under the Short Term Disability Policy, a "Year" as it relates to Length of Service is defined as
        a full year of employment, measured backward from the date short term disability begins to
        be applied.
        An employee is not required to exhaust Sick, Vacation, Floating Holiday or Personal leave
        before receiving Short Term Disability payments except as described in the previous section
        entitled Waiting Period. While on Short Term Disability leave accruals will not be suspended.
        However, first Sick leave, then Vacation, Floating Holiday, or Personal leave must be used as
        balances allow to enable an employee to receive 100% pay for as long as possible during the
        duration of their disability.
        While on Short Term Disability benefits, employees will maintain continued eligibility in all
        benefit plans and programs they were enrolled in immediately prior to their period of
        disability.

        Benefits under this program will be coordinated with NWF's Family and Medical Leave Policy.
        If an employee is entitled to disability benefits from the NWF sponsored Worker's
        Compensation insurance program, benefit payments under this Short Term Disability Policy
        will not be made.



        In order to be eligible to receive benefits under the Short Term Disability Policy, the employee
        should report their absence and the nature of the illness, injury, or medical condition to their
        Supervisor or their Supervisor's designate and the Human Resources Department.  Future
        disability situations which are pending should be brought to the supervisor's attention as soon
        as determining facts are known.



        After notification, the Human Resources Department will provide the employee with the
        proper form(s) which must be completed by their personal physician indicating nature of the
        disability and expected duration.  Periodic reports from the physician will be required in order
        for payments to be continued during an extended period of disability.  In addition, a physical
        examination or evaluation by a physician designated by NWF may be required during the

        absence or to approve the employee's return to work.  The expense of any such examination
        will be paid by NWF.
