                   6.  Enter the Effective From date (this first date should be today’s date), select Continue Request on the far
                       right-hand side of the screen









                   7.  Enter the termination date (this needs to be the last day the employee physically worked – obtained
                       prior to starting the termination process)















                   8.  Notate if the employee has provided notice
                          •  If no notice given, check the No Notice box















                   9.  Select the Termination Reason drop-down, select the appropriate termination reason















                   10. Enter the PTO Balance in hours:minutes format (obtained prior to starting the termination process)
