              If an employee’s disability makes it difficult for him/her to comply fully with the Company dress code, we may be
              able to provide reasonable accommodations.
              Please contact your Human Resources department regarding any questions you might have.

              HAIR AND TATTOO GUIDELINES

              HAIR

               •   Hair should be neat and clean. Hair color of an unnatural tone to include unnatural tones of natural colors is
                   not permitted (green, blue, purple, etc.)
               •   If an employee’s disability makes it difficult for he/she to comply fully with a dress code, the employee should
                   speak to Human Resources about a reasonable accommodation. Any question regarding the dress code policy
                   should be directed to your Manager or the Human Resources Manager.

              TATTOOS

               •   Tattoos must be small in size, minimally noticeable, not on the face, non-controversial (not advocating an interest),
                   and inoffensive to customers, visitors, families, and other employees. Employees that object to these guidelines
                   must cover tattoos during work hours.



            6.3  SAFETY GUIDELINES

              A-MAX Auto Insurance believes in maintaining safe and healthy working conditions for our employees. However, to
              achieve our goal of providing a safe workplace, each employee must be safety conscious. We have established the
              following policies and procedures that allow us to provide safe and healthy working conditions. We expect each
              employee to follow these policies and procedures, to act safely, and to report unsafe conditions to his or her supervisor
              in a timely manner.

               6.3.1  REPORTING UNSAFE CONDITIONS OR PRACTICES

               Employees are expected to continually be on the lookout for unsafe working conditions or practices. If you observe an
               unsafe condition, you should warn others, if possible, and report that condition to your supervisor immediately. If you
               have a question regarding the safety of your workplace and practices, ask your supervisor for clarification.

               If you observe a coworker using an unsafe practice, you are expected to mention this to the coworker and to your
               supervisor. Likewise, if a coworker brings to your attention an unsafe practice you may be using, please thank the
               coworker and make any necessary adjustments to what you are doing. Safety at work is a team effort.
