Living Will

This document enables you to express now, while you are able to do so,
what your preferences are regarding the use of artificial life-sustaining
measures in the event certain end-of-life crisis conditions are met and
you are unable to express your wishes at that time. It eliminates the
burden on your loved ones of having to guess what you would want under
the circumstances and helps ensure that your preferences are honored.

HIPAA Release

If you aren't familiar with HIPAA, it is a federal law that is related to the
privacy of your health information. It's a very overbearing law, so this
Release ensures that the person making medical decisions for you has
access to your medical information.

Declaration of Pre-Need Guardian

Ideally, the other Advance Directives in unison will avoid the need for
guardianship in the event of your incapacity; however, if for some reason
you should be determined by a court to be in need of guardianship, the
person named by you in this document will have the absolute priority in
appointment as guardian so long as they are qualified to serve. This
eliminates arguments among your loved ones as to who should be
appointed as your guardian.
