                   F. Assigned to (the name of the person the information came
                   from; A dot is placed if it is requested by the guest)
                   G. HDO (Help Desk Officer on duty)
                   H. Remarks (other details; a dot as default)
                   e.g. Number of pax in a newly checked in room

                   Information logged in the Inquiry Panel:
                   a. Outside Calls
                   b. Crisis
                   Details:
                   A. Name of caller/ company
                   B. Description (Guest inquiry/ Transfer call)
                   C. Remarks

                    4.  For Request Panel, mark as Relayed once you have
