VOLUNTARY CRITICAL ILLNESS
100% paid by the Employee
Full-time employees may join the critical illness insurance plan on the first of the month
following their date of hire.

VOLUNTARY CANCER INSURANCE
100% paid by the Employee
Full-time employees may join the cancer insurance plan on the first of the month following
their date of hire.

Upon termination, you may be entitled to continuation or conversion of the group
voluntary cancer insurance plan in accordance with the terms of the policy and/or
applicable state law.
SHORT TERM DISABILITY INSURANCE
100% paid by the Employer.
Regular full-time employees are provided short-term disability insurance in the event of
a non work-related sickness or injury resulting in total disability. Sickness shall include
pregnancy, childbirth and miscarriage. Coverage begins on the first day of the month
following the date the employee became a regular full-time employee. The plan
provides for a benefit of 66-2/3% of base wages or salary up to a maximum benefit of
$1,000 per week. If the employee is paid hourly, the benefits are based on an eight-hour
workday. Benefits begin on the first day of the accident and the 8th day of the disability
due to sickness or pregnancy and continue up to a maximum of 180 days. Unless periods
of total disability are separated by your return to active work for at least two consecutive
