SUPPLEMENTAL LIFE INSURANCE AND ACCIDENTAL DEATH AND
                 DISMEMBERMENT
                 100% paid by the Employee.

                 Regular full-time employees working at least 32 hours per week have the option to
                 purchase  additional  life  insurance  100%  paid  by  the  employee.    See  Human
                 Resources for plan details and options.

                 24 HOUR ACCIDENTAL
                 100% paid by the Employee

                 Full-time employees may join the accidental insurance plan on the first of the month
                 following their date of hire.

                 The Summary Plan Description (SPD) containing details of the plan and eligibility
                 requirements may be obtained from Human Resources.

                 Upon termination, you may be entitled to continuation or conversion of the group
                 accidental  insurance  plan  in  accordance  with  the  terms  of  the  policy  and/or
                 applicable state law.

                 VOLUNTARY CRITICAL ILLNESS
                 100% paid by the Employee

                 Full-time  employees  may  join  the  critical  illness  insurance  plan  on  the  first  of  the
                 month following their date of hire.

                 The Summary Plan Description (SPD) containing details of the plan and eligibility
                 requirements may be obtained from Human Resources.

                 Upon termination, you may be entitled to continuation or conversion of the group
                 critical  illness  insurance  plan  in  accordance  with  the  terms  of  the  policy  and/or
                 applicable state law.

                 VOLUNTARY CANCER INSURANCE
                 100% paid by the Employee

                 Full-time  employees  may  join  the  cancer  insurance  plan  on  the  first  of  the  month
                 following their date of hire.

                 The Summary Plan Description (SPD) containing details of the plan and eligibility
                 requirements may be obtained from Human Resources or found within Paycor.
