SECTION 125 PLAN
Your Employer offers a pre-tax contribution option for employees. This employee
benefit is known as a Section 125 plan.
A Section 125 plan is a benefit plan that allows you to make contributions toward
certain qualified benefits on a pre-tax rather than an after-tax basis. This means your
qualified expenses are deducted from your gross pay before income taxes and Social
Security are calculated.
By joining the Section 125 plan, you elect to have your gross pay reduced by
qualified benefit premium amounts. Section 125 laws must be followed. See plan for
details.
COBRA
You and your covered dependents will have the opportunity to continue medical,
dental and/or vision benefits for a period of up to 36 months under the provisions of
the Federal Consolidated Omnibus Budget Reconciliation Act (COBRA) when group
medical, dental, vision and Allstate coverage for you and your covered dependents
would otherwise end due to your death or because; your employment terminates,
for reason other than gross misconduct; Your employment status changes due to a
reduction in hours; your child ceases to be a “dependent child” under the terms of
the medical, dental and vision plan; you become divorced or legally separated; or
you become entitled to Medicare; in the event of divorce, legal separation, or a
child’s loss of dependent status, you or a family member must notify the plan
administrator within 60 days of the occurrence of the event.
The plan administrator will notify the individuals eligible for continuation coverage
of their right to elect COBRA continuation coverage.
