                                                              Article II

                                                       ADMISSION POLICIES


                        I. Admission Guidelines & Procedures for Level I

                              The  College  of  Nursing  coordinates  with  the  University  Admissions  in  the  screening  of
                          applicants who are graduates of K-12. The University Admissions Office administers Cagayan
                          State  University-College  Admission  Test  (CSU-CAT)  and  Evaluation  of  Secondary  School
                          documents while the College of Nursing Faculty will conduct the Interview.

                          A. Applicants to the Nursing Program must:

                             1. Be a Senior High School graduate from the K-12 program, preferably under the STEM
                                 Strand of the Academic Track.


                             2. Submit the following entrance documents to the Screening Committee:

                                a. Certified  true  copy  of  the  Senior  High  School  Card  indicating  a  General  Weighted
                                 average of at least 85%
                                b. College Admission Test result of a minimum 88%
                                c. Certificate of Good Moral Character from the head of school/guidance counsellor
                                d. NBI Clearance or Police clearance
                                e. Two copies of passport sized ID picture (colored with name tag and white background)
                                f. Birth certificate from PSA / authenticated by PSA
                                g. Marriage certificate (if applicable)
                                h. Non-reactive Hepatitis-B Serum antigen test result.
                                i. Negative drug test result


                              3. Pass the interview to be conducted by the designated Program Screening Committee.
