Witness details
It is very important to obtain names and addresses of any witnesses along with contact
telephone numbers. Disputed road traffic crashes can often depend upon the evidence of
independent witnesses. It is essential that as much information as possible is obtained.
Police
It should be noted that, if there has been any injury or any damage to property, it is an
offence not to report an incident so please make sure you report it as soon as possible – in
any event within 24 hours. When the Police attend the crash / incident, a record of the
attending officer(s) name and number should be obtained. In the case of theft or malicious
damage, a crime reference number needs to be obtained from the Police as soon as
possible.
Incident Checklist
On the next page you will find an Incident Checklist. Please complete this at the scene
if possible.
