a. The pattern of proposed land use including shape, size and
                         arrangement of proposed use areas density and environmental
                         character and the relationship to surrounding uses.

                b. The pattern of public and private streets, pedestrian ways and
                         parking areas proposed and the relationship to existing streets.

                c. The location, size and character of recreational and open
                         space areas designated for private reservation or reserved or
                         dedicated for public uses such as school, park, greenway, etc.,
                         or any other special amenity to be provided.

                d. A utility feasibility study including the general system of
                         sewer, water and storm drainage.

                e. A general analysis of the impact of the development upon
                         traffic facilities.

        4. Appropriate statistical data on the size of the development, ratio of
                various land uses, percentages of multiple-family units by number of
                bedrooms, economic analysis of the development, expected staging
                and any other plans or data pertinent to proper evaluation of the
                proposal.

        5. General outline of intended organizational structure related to
                property owner’s association, deed restrictions and private provision
                of common services.

        6. Any additional information requested by the Plan Commission as
                being necessary for adequate evaluation of the concept plan,
                including completion of any required application forms or checklists.

(d) Referral and Hearing.

        1. Within 60 days after the filing of the petition for approval of a
                general development plan is complete, as determined by the Zoning
                Administrator, the Plan Commission shall forward the petition to the
                Village Board with a recommendation that the plan be approved as
                submitted, approved with modifications or disapproved. Such report
                shall include findings of fact specifying the reasons for the Plan
                Commission’s recommendation.

        2. Upon receipt of the Plan Commission’s recommendation the Village
                Board shall determine whether or not to initiate a proposed zoning
