401(k) Retirement Savings Plan Fidelity
Mission sponsors a 401(k) Retirement Savings Plan through Fidelity Investments. It allows eligible participants to
contribute up to the 2022 IRS maximum. Additionally, there is also an after-tax Roth option available. Effective
November 1, 2021, Mission will match 25% of an employee’s deferrals not to exceed the annual IRS maximum. Mission
does not match the $6,500 catch-up contribution for those age 50 or older. All employer contributions are subject to a 6
year/20% graded vesting schedule beginning at the employee’s second year of service. Employee contributions are
always 100% vested.
Plan Eligibility & Entry
The Plan is open to all employees immediately as of their date of hire. Participants may increase or decrease
contributions on any pay period. Contribution percentages can be changed by participants by either contacting Fidelity at
800-835-5097 or logging into www.401k.com.
Beneficiary Information
You are required to set up your beneficiary information. Do so by going to Fidelity at www.401k.com or calling a
Fidelity Representative at 800-835-5097 to request a Designation of Beneficiary Form to fill out and return to Fidelity.
Investment Options
Participants may choose to invest their balances among several different fund options. Participant accounts are
automatically set up with Fidelity Investments and are accessible after the participant’s first contribution is made.
Participants can make investment elections by logging into their account at www.401k.com.
Questions
If you have questions about your account, you can speak with a Customer Service Representative by calling 800-
835-5097.
ERISA Section 404(c) Compliance
The Plan is intended to be an ERISA Section 404(c) plan. This simply means that you "exercise control" over some
or all of the investments in your Plan account. The Fiduciaries of the Plan may be relieved of liability, or responsibility,
for any losses that you may experience as a direct result of your investment decisions.
Please review your current deferral election and determine whether or not it needs to be changed in order for you to
reach your stated maximum for the year.
You can log on or call to inquire about your account balance, transfer investments or to change future
investment selections.
Participants can specify or change their deferral election by logging onto the Fidelity website at www.401k.com or by
calling customer service at 800-835-5097.
