Short & Long Term Disability Unum

Mission provides full-time employees with short and long-term disability income benefits and pays the full cost
of this coverage. In the event you become disabled from a non work-related injury or sickness, disability
income benefits are provided as a source of income. Disability is subject to the approval of Unum after review
of physician statements.

Disability benefits may be offset by income you receive from workers’ compensation, Social Security, or other
disability coverage.

Short Term Disability
You are provided with Company-paid short-term disability (STD) coverage. The plan provides
coverage of 60% of your average gross weekly income as figured from the income box on your W-2
up to $2,500 per week after a 14-day disability elimination period for a maximum of 25 weeks. For
employees who do not have a full calendar year of service the year prior to disability, or that are hired
in the year of disability, “weekly in- come” means your Annual Salary in effect just prior to your date
of disability.

Long Term Disability
You are provided with Company-paid long-term disability (LTD) coverage. The plan provides coverage
of 60% of your average gross monthly income as figured from the income box on your W-2 up to
$12,500 per month after 180 days of short-term disability. Benefits end when you are no longer totally
disabled or when you reach your SSNRA (Social Security Normal Retirement Age), whichever occurs
first. For employees who do not have a full calendar year of service the year prior to disability, or that
are hired in the year of disability, “monthly income” means your Annual Salary in effect just prior to
your date of disability.
