Family Medical Leave (FMLA) Unum
What is FMLA?
The Family and Medical Leave Act of 1993 provides up to 12 weeks, in any 12 months, of unpaid, job-
protected leave for the birth or adoption of a new child, for an employee’s own serious medical
condition, to provide care for a family member (child, spouse or parent) with a serious medical
condition, or in situations of qualifying exigency to be with an employee’s spouse, parent or child if
they are an active military service member or have an impending call to active duty in support of a
contingency operation.
In addition, up to 26 weeks of leave can be granted in a single one year time period for the care of an
injured service member, if the service member is the employee’s spouse, child parent, or “next of
kin”. FML may be taken on a continuous, intermittent or reduced schedule basis.
To file a request for FML, please call UNUM at 866-779-1054. Note: If your FML is due to a qualified
disability, UNUM will transition your request to the short term disability claim professionals who will
verify your eligibility and initiate the claims process.
