•  Physical Assessment:

                              o  Assess for the following clinical manifestations:

                       •  Elevated temperature 38.5°C chills.

                       •  Foul-smelling lochia.

                       •  Abdominal tenderness, pelvic pain.

                       •  Dysuria, burning on urination.


                       •  Tachycardia.

                       •  Increased WBC.

                       •  Presence of predisposing factors:

                       •  Traumatic delivery.

                       •  Prolonged difficult labor.

                       •  Dehydration.

                       •  Excessive vaginal exams.


                       •  Premature rupture of membranes.

                       •  Anemia.

                       •  Retained placental fragment.

                       •  Hemorrhage.



                   Psychosocial/Cultural Assessment:

                       •  Mother experiences frustration associated with extreme fatigue and she

                          may appear uncooperative and uninterested.

                       •  Planning /Goals:

                              o  Provide safe, effective environment.

                              o  Provide teaching regarding hygienic measures.

                              o  Promote physiological integrity.

                              o  Administer treatment specific to infection.

                              o  Promote psychosocial integrity.
