FORMED is a manufacturer of high-quality, innovative products in the field of specialized medical
equipment and furniture. We manufacture them according to Quality Management System ISO-9001 and
ISO-13485. Formed products meet requirements of the European Medical Device Directive 93/42/EEC and
are CE marked.

Among products offered for markets are SYMBIO furniture as a medical furniture dedicated for doctor’s
and nurse’s rooms and for patient rooms as well. They met requirements established by the Polish
Institute of Hygiene. Our slogan is: SYMBIOmed We have a heart for furniture.

The SYMBIO line was created for the overall furnishing of the hospital, taking into account all types of
rooms. A hospital, like any other medical facility, is a space with a special and complex purpose, and the
furniture line we have created includes all the required elements: from specialist equipment to the
smallest furniture details. The SYMBIO collection is supplemented with numerous equipment options that
improve the use of the product, dedicated to the medical service. In the category of SYMBIO furniture,
we distinguish medical, aluminum, steel and laboratory furniture.

Comprehensive furnishing of the facility with furniture and medical equipment is a particular challenge.
As part of our comprehensive equipment service, we offer the preparation of a real action plan,
preparation of the arrangement and offer, as well as equipment of the facility with the assembly of the
delivered furniture.

We manufacture our products according to Quality Management System ISO-9001 and ISO-13485.
SYMBIO furniture comply with requirements of the Polish Institute of the Hygiene. We are flexible team
