                            concerns, symptoms, and special requests.


                        b   The physician and patient review and budget visit
                            time appropriately.


                        b   If necessary, follow-up is scheduled to address less
                            urgent items.

                        b   It is a concise summary outlining the key points, plan,
                            medication changes, and follow-up instructions.



                        b   Patients can review the information in the less
                            stressful home environment.



                        b   This can be shared with other caregivers.
