Communication
As we learned from the earlier graph, 64 percent of SVMIC’s
closed malpractice claims in obstetrics involve a breakdown
in communication. The following graph shows the types of
communication failures that occurred:

We identified that 41 percent of the failures occurred between
a physician and the patient, 35 percent occurred between
the physician and staff, and another 22 percent of the failures
occurred between the physician and other physicians. As
