         Job Description
         POSITION  SUMMARY:  Independent  Medical  Group,  LLC  (IMG)  specializes  in  the  testing,  treatment,  and
         management of HIV, Hepatitis C, and Sexually Transmitted Diseases. Co-founders Clifford W. Knights and Steve
         Vixamar have built a partnership with the Centers for Disease Control and Prevention (CDC) to level the playing field
         for all high-risk Americans, no matter their race, sexual orientation, or socioeconomic status. IMG offers an array of
         services that includes healthcare and pharmacy services, case management, mental health, and substance abuse
         management.  We  are  looking  for  highly  committed  Patient  Outreach  Drivers  to  engage  with  underserved
         individuals in our community. Our Patient Outreach Drivers identify, engage, and transport individuals in need of
         our clinics so they can receive HIV awareness and prevention education and ensure they are provided STD and HIV
         testing, counseling, and referrals. This program provides risk reduction awareness and advocacy and a different
         approach to life through street outreach, intervention counseling, and HIV prevention education.

         ESSENTIAL DUTIES & RESPONSIBILITIES:

             The job duties listed below are representative of the essential job functions and are not exclusive. Duties,
             responsibilities, and activities may change, or new ones may be assigned with or without prior notice.

             ▪  Conducts street and community outreach
             ▪  Recruits participants for testing, treatment, and counseling services
             ▪  Engages street contacts in risk reduction discussions
             ▪  Gives out HIV/HEP C/STD risk reduction information
             ▪  Makes home visits and reaches out to persons/patients that present an adherence problem to treatment
             ▪  Ensure data collection in a timely, accurate, and consistent manner
             ▪  Distributes preventive materials, including safe sex kits
             ▪  Provide Outreach and Recruitment activities including but not limited to representing the program and the
                 agency at community and committee meetings, providing educational workshops and social marketing
                 activities

         OTHER DUTIES:


             ▪  Performs other duties and responsibilities as assigned by your supervisor

         COMPUTER EQUIPMENT & SOFTWARE SKILL REQUIREMENTS:


             ▪  Full knowledge of Windows Office Systems, particularly Microsoft Word, Excel, and Outlook.
             ▪  The ability to utilize Google Drive and Gmail for sending emails
             ▪  Full knowledge of EMR/EHR systems and telehealth.
             ▪  Use of scheduling software

         MINIMUM REQUIRED QUALIFICATIONS & EXPERIENCE:

             Education:  A high school diploma or general equivalency diploma (GED) is required.


             Experience:       A minimum of one (1) year of professional work with underserved individuals who are
             potentially HIV Positive, drug addicts, or working with prevention of STD’s, is required.
