Section 4. Procedures in Claiming Benefits



     a. Submit to the Office of the Student Affairs and Services (OSAS)

        a written report of the accident and same shall be forwarded to

        the Accounting Office for the preparation of                               disbursement

        vouchers.

        b. Submit required documents such as the following:




          b.1. In case of death due to natural causes:


             ✔ Death Certificate


             ✔ Certificate of Enrolment signed by the Registrar

      b.2. In case of accident (not related to school activities):



             ✔ Police report/Barangay report


             ✔ Medical Certificate & Hospital Bills


             ✔ Certificate of Enrolment signed by the Registrar



      b.3. In case of accident (related to school activities):



          ✔ Written reports of the Office of the Student Affairs and

             Services or University officials                    (i.e. Faculty spot report,

             Dean’s report, Adviser’s report or Project In-Charge

             report, etc.).

             ✔ Medical Certificate & Hospital Bills.


             ✔ Certificate of Enrollment signed by the Registrar.


          ✔ Other documents as maybe required to compliance to the

             existing accounting and auditing rules and regulations.



   Note: A separate account shall be opened in the university official
   depository bank where the amount collected for the mutual aid
