A student who commits a mistake or error while rendering patient care shall be subjected to the College of
Nursing Investigating Committee, a committee created to investigate matters regarding students’ conduct
inside the school and in the clinical areas when misdemeanor needs further, through deliberation and
proper action.
The Investigating Committee, as stated in the ISU Student Handbook, is composed of the
following: Chairman and least two members (one is the President of the Student Council). The Dean, Over-
all Coordinator, or Executive Officer at the start of the school year shall designate all other members.
Disciplinary Proceedings
1. Once a student commits an error in the clinical area, the student and the Clinical Instructor supervising
him must submit to the Dean an Incident Report signed by Head Nurse. The Dean then forwards the report
to the Investigating Committee, which shall determine if the report is sufficient to warrant an investigation.
2. The College of Nursing Investigating Committee shall follow the disciplinary proceedings stated in the
Student Handbook with regard to filing of charges, preliminary inquiry, notice of hearings and others.
3. Parents or guardians are informed about the complaint either through a letter or verbal invitation for
dialogue with the Dean, College of Nursing Investigating Committee and the Guidance Counselor.
4. The sanction for an offense, circumstances to its commission and frequency of the sanctions below
should be applied:
a. Expulsion from the College of Nursing.
b. Suspension of not more than 20% of the prescribed number of school days
c. Extension of duty
d. Demerit
e. Reprimand
Misdemeanors
1. Error in the administration of medication treatment (failure to observe the R’s in administering
medications, giving medications prepared by another student, recording medications given by another
student, improper recording of medication and treatment in the patient’s)
2. Taking home the patient’s chart.
3. Leaving the area without permission or proper endorsement by authorized persons.
4. Writing false or incorrect entries on the patient’s chart.
5. Reporting for make-up duty without proper endorsement.
6. Loitering in the clinical area and engaging in vandalism or gambling
7. Sleeping while on duty.
