a. Students doing scandalous and promiscuous acts shall be sent home and referred to the
Guidance Office for proper action.
b. Students should comply with other policies set by the dorm management and faculty in charge
c. Incident Reports should be written by the concerned student immediately and submit to the
faculty in charge
d. Students sent home because of violating dorm policies will not be given a make-up duty.
Hospital
Operating Rooms/ Delivery Rooms
1. Students must go to the hospital in their duty uniforms. They only wear their OR/DR uniform
inside the OR/DR, which consists of OR/DR suit with camisole for females and males, cap,
mask, nameplate and rubber slippers.
2. When students go out of the OR/DR like going to the ward or for a break, they must wear a
gown over the OR/DR suit and change their slippers.
Procedures for Make Up
1. A student must present to the Clinical Coordinator the following documents in order to secure a
make-up slip:
a. For Absences: excuse letter duly signed by the party and addressed to the Clinical
Instructor concerned and Admission Slip duly signed by the Over-all Coordinator and the
Dean. Attachments must accompany excuse letter. The Clinical Coordinator determines
the number of hours to be made up based on the remarks of the CI in the excuse letter.
b. For Demerits due to Incomplete Paraphernalia or Tardiness of not more than two times:
excuse letter addressed to the CI concerned with corresponding remarks on the number of
hours to be made up.
2. The students must go to his Level Coordinator for the schedule of make-up.
3. After the student has made up the lost number of hours, he/she must present his duly
accomplished make-up slip to his clinical Instructor under whom the absence was made.
4. Students must go for duty on their scheduled make-up. Failure to do so means another
absence was made.
Health Services
A student who is sick(with beginning or frank signs of communicable disease or with other
health conditions that may predispose him/her or the patient or other people to disease)
shall not be allowed to render duty.
Sudden illness while on duty must be reported to the Clinical Instructor. The CI must send
or refer the student to the hospital or the Infirmirary or to the nearest health agency as
soon as possible.
