2.  Absences

            1.  Absences can be excused or unexcused as stated:
                       Excused
               a. Force majeure
               b. Death of direct relative
               c. Got sick with medical certificate from the University doctor
                       Unexcused
               a. Absent without medical certificate from the University doctor
               b. Other reasons aside from excused absences

            2.  A student must complete the prescribed number of RLE hours per level. Failure to do so because
               of  inability  to  make  up  for  lost  hours  in  case  of  absences  mean  a  grade  of  70  in  the  area  of
               exposure.

            3.  An  absent  student  upon  reporting  to  his  Clinical  Instructor  must  present  an  excuse  letter  duly
               signed by the Dean and over-all Coordinator

            4.  Excused  and  unexcused  absences  shall  be  made  up  within  two  or  three  weeks  unless
               circumstances  are  unfavorable.  If  the  absences  are  not  made  up  within  the  prescribed  time,  a
               grade of 70 is given to the student.

            5.  A student who attends co-curricular or extra-curricular activities shall not make up for lost hours but
               he shall be given additional assignment to develop the expected competency/competencies.

            6.  Guidelines for make-up duties:

        Tardiness:
            1.  Tardiness after 1-15 minutes after the scheduled time is equivalent to 1 demerit
            2.  Tardiness of more than 15 minutes after the scheduled time is equivalent to 1 unexcused absence
               corresponding to 3 days make-up duties.
            3.  3 demerits or 1 excused absence is equivalent to 1 make up duty.

        Absences:
        One (1) Unexcused absence-------------3 days make-up duties
        One (1) Excused absence-----------------1 day make-up duty

            4.  Payments for the make-up duty will be shared by the students to meet the per hour rate of the
               clinical instructor

            3.  Uniform
               Hospital
              1.  Students must wear the prescribed hospital uniform with apron. Uniform should be clean and well
                 pressed.
              2.  Students must wear their nameplates all the time.
