Example of recently added Coordination Note
To ensure that all patients get the best care at the time of their call to VNSNY, the CCC RN will continue
to perform clinical assessments and triages for all patients calling the Customer Care Center with
symptom-based issues.
What will change?
• The clinical triage note will be embedded in HCHB.
• The Branch will no longer receive a separate copy of the clinical triage note.
How will the triage be documented?
The CCC RN will document directly into the EMR in one of the two CCC clinical notes below:
• CCC Urgent Clinical Disposition
• CCC Coordination Clinical Call
Clinical Field Manager Responsibility:
• Review the Coordination Notes on their worklists and follow up with the appropriate action.
New Coordination Note- Update Patient Authorization. Patient Authorization Note will route
back from RCD to the Scheduler
