- In the office, the scheduler did not take the correct action and the visits need to be
                        researched and rescheduled.


               Reports used in CRM Review

               a. Home Health Case Mix-STEP 3 (MEDICARE only) CRM may look at gross margin% and compare the
               avg. #visits with what is scheduled.  If % is low, and visits exceed clinical pathways, see if adjustment in
               visit frequency is warranted. If CRM decreases visit frequency, look again and see if gross margin%
               increases.
               b. Episode Analysis Tool: Currently grayed out but CRMs may be looking at this in the future.

               c. OASIS Therapy Report: If rehab services are ordered and CRM is unsure if clinically warranted, look at
               score to confirm need for rehab evaluation.  If a rehab evaluation is Not ordered but CRM suspects
               patient may benefit, look at the score and recommend PT evaluation if clinically needed. Review scores
               at the bottom of the last page. Then confirm with SOC clinician that patient may benefit from rehab
               eval. Adjust orders as needed.
               d. Home Health Case Mix Details and Insight Report: CRM may use to see how changing score on OASIS
               impacts HHRG score.

                                 •  This report is used by Branch Directors and CFMs
                                 •  Report includes  patients who have not been seen in 14 day or more
                                 •  Also includes SOC visits that has not been assigned
                                 •  These cases are open past their episode end date
                                 •  Sort by Branch and teams
                                 •  The Branch Director/CFM are responsible for reviewing the cases on the
                                     report.
