CHHA Branch Model – One interdisciplinary team
Branch Model With clinicians at the center of operations, branches and
Census 500 centralized clinical functions will work to support,
coordinate and oversee home health services. One Branch
Director will head each branch, a combination of two
existing teams. Other branch roles include:
