                                          DOH Statement of Deficiencies and Plan of Correction
                           AREA OF DEFICIENCY                         CLINICAL MANAGER OVERSIGHT
                MD Communication                       Any changes in patient condition must be reported to MD
                Adherence to visit Plan of Care visit frequencies  -Missed Visits
                                                       -Any and all changes to the original Plan of Care
                Medication Management                  -Review medication order
                                                       -If case is a PT SOC, CFM MUST complete the medication reconciliation
                HHA supervision                        -Ensure there is a supervisory visit plotted (RN10 or PT10)
                                                       -MD notification of delays in placement of HHA
                                                       -Encourage the field clinicians to have the paper plan of care match the
                                                       plan of entered into the tablet, including any changes made throughout
                                                       episode
                Discharge Planning                     -Discharge planning with patient and family starts on Day 1
                                                       -Medicare notices must be given timely
                                                       -All relevant physicians must be informed of agency/discipline
                                                       discharges
                OASIS Accuracy                         -CRMs should be contacting the field clinicians prior to make changes to
                                                       OASIS assessment items.
                                                       -Field clinicians should be responding promptly to the OASIS alerts
                                                       -Upon learning of a patient being hospitalized, the clinical team MUST
                                                       communicate to determine who is going to complete the TIF

                •Data is released quarterly. The most recent report
                  displayed in October 2018 is  based on OASIS data from Jan
                  1, 2017 to December 31, 2017. The report suggests that

                  VNSNY’s current overall quality measures rating remains at
                  3.0 STARS.
