                           Service Delivery


                           1.  Referral Report- daily
                              Ensure SOC delivered timely
                           2.  Agent Summary Report- daily
                              Monitor late or missing visits. Address potential LUPA.
                           3.  Field Staff Productivity by Worker Report- weekly
                              Use to ensure fair caseload assignments
                           4.  Recerts Due by Case Manager Report- weekly
                              Monitor and manage episodes nearing their end for appropriate recert/ discharge decision.
                           5.  Active Episode Visit Tracking (dashboard) - twice weekly;
                              Track visit activity such as no visit in 14 days. Sort by case manager for bin reviews. Track
                              active cases with no scheduled Discharge.
                           6.  Agent Last Renewal and Refresh Report - weekly
                              Monitor clinician compliance with performing a “Selective Refresh” every Wednesday.  Also
                              displays last Sync date
