                           1. Pick your own Branch and Team (optional for Case Manager)










                            2. Click on Last completed Visit and service code – Sort by Ascending

                           Active Episode Visit Tracking:
                            Purpose




                           Allows the Clinical Manager to:
                            Track visits and identify cases with no visits in >14 days
                            Track cases for timely discharge
                            Track cases without Case Manager
                            Track to confirm discharge and recert visits are plotted on the patient’s
                              calendar
                            Track cases with unassigned appointments


                           Notes:
                           o Run daily or as needed
                           o Dashboard information is updated every 4 hours
