CHHA QAPI Plan
• The CHHA Quality Assurance and Performance Improvement (QAPI)
program supports the mission, vision and guiding principles of VNSNY
by promoting compassionate patient-centered care that is safe,
efficient, effective and timely.
• The QAPI program is data driven and promotes the incorporation of
evidence-based practice into care delivery to achieve the best
possible outcomes for patients.

CoP requirements for data-driven QAPI Program
• Quality Assurance (QA) = the process of meeting quality standards and assuring
that care reaches an acceptable level
• Performance Improvement (PI) = continuously analyzing your performance and
developing systematic efforts to improve it; also known as Quality Improvement
Focuses on indicators related to improved outcomes and preventions
Measure, analyze, and track quality indicators, including adverse patient events,
and other aspects of performance that enable agency to assess processes of
care, services, and operations
Must at be capable of showing measurable improvement in indicators in health
outcomes, patient safety, and quality of care.
Performance improvement activities, track adverse patient events, analyze their
causes, and implement preventive actions.
