Heart Failure

We created a specialized program for our patients with heart
failure. Our goal is to help patients manage their condition
by utilizing the most up-to-date research, evidence-based
guidelines and our years of experience caring for people
with heart failure. Depending on the individualized plan
we develop with our patients, home care services may
include nursing, physical therapy, occupational therapy,
speech therapy, social work, telehealth services and a
home health aide.
Our professional staff can help patients follow their doctor’s
recommendations and learn to manage heart failure at
home. Our services include:
• Helping patients to take medications properly and monitor

their effects
• Helping patients to make necessary modifications to
their diet to control their blood pressure, cholesterol, and
weight
• Teaching patients to recognize the signs and symptoms
that should prompt them to seek emergency care
• Teaching patients how to use a home blood pressure
