VNSNY Chronic Care Expertise
Rehabilitation
Our highly trained physical therapists, occupational therapists
and speech language pathologists work with patients to
develop goals for treatment and coordinate with other
medical providers—like doctors and nurses—to deliver a
plan of care that meets their individual needs.
Our therapists utilize evidence-based practice, work closely
together to provide the best care possible to quickly
improve a patient’s function, improve safety and promote
independence as much as possible. Specifically:
• Physical Therapy provides hands-on care to improve a
patient’s strength, flexibility, balance, and coordination.
There is special focus on improving ADLs and decreasing
falls risk
• Occupational Therapy provides hands-on care to improve
ADLs and to address low vision and cognitive deficits
• Speech Language Pathology develops programs to
help patients strengthen the muscles of the mouth,
neck, and face to alleviate problems such as choking
and coughing, while improving the ability to speak clearly,
chew, and swallow. They also address language-related
cognitive deficits to improve communication
and processing.
