Ns. Yulia    :  “So, you’ve been vomiting since last night. Do you also have diarrhea since having the
                                IV?”
                Mrs. Ana     :  “No, I don’t.”
                Ns. Yulia    :  “That’s good. Would you mind if I do the IV Catheter site assessment now?”
                Mrs. Ana     :  “No problem.”
                Ns. Yulia    :  “All right, let me check your arm. Oh, the IV bandage looks wet, I think you may have
                                had leakage or some sort of infiltration of the IV fluid to the surrounding site insertion
                                tissue. Would you mind if I open the dressing to check if there is really an infiltration?”
                Mrs. Ana     :  “No, please open it if necessary.”
                Ns. Yulia    :  “I didn’t see any blood clots in the IV hub and tube though. Excuse me, I’m going to
                                palpate your wrist area around the IV. Tell me if it hurts.”
                Mrs. Ana     :  “Oh, yeah… it hurts a bit there.”
                Ns. Yulia    :  “Hmm…. You have a little swelling here, but I see no redness, skin discoloration or bluish
                                tint, or cord-like hardness on the site. But, here I can see that there is dislodgement of
                                the IV catheter. The IV tube isn’t all in the vein.  Does it hurt only when it’s touched or
                                it hurts all the time?”
                Mrs. Ana     :  “Well, when you palpate it just now, it hurts. But, I wouldn’t say it hurts all the time. It’s
                                just a bit uncomfortable around the needle area.”
                Ns. Yulia    :  “How does it feel?”
                Mrs. Ana     :  “I’m not sure how to describe it. It often feels like a pulsing sensation that comes and
                                goes quickly.”
                Ns. Yulia    :  “Oh I see, like a throbbing sensation. Do you also feel numb, tingling or a bit of a pricking
                                sensation?”
                Mrs. Ana     :  “Ah… yeah. It feels like a prick. But it’s not that bad. It is still bearable.”
                Ns. Yulia    :  “I see… it also feels tender when I touch it. Do you also feel hot or warm around the
                                area?”
                Mrs. Ana     :  “A bit yeah…”
                Ns. Yulia    :  “Does it also itch around the area?”
                Mrs. Ana     :  “No, it’s not itchy.”
                Ns. Yulia    :  “All right, Mrs. Ana. I’ve noted all of the symptoms you’re having. The IV catheter is
                                dislodged, and you also have an infiltration. So, I will remove the IV catheter then I will
                                put a compress on it and hope you will feel more comfortable.”
                Mrs. Ana     :  “Sure, I appreciate it.”
                Ns. Yulia    :  “No worries, you’ll get better soon.  Allahumma rabban nasi, adzhibil ba'sa isyfi antas
                                syafi la syafiya illa anta syifa'an la yughadiru saqaman.”
                Mrs. Ana     :  “Amen, thank you so much.
