Getting Familiar with Patient’s Discharge Plan

                  Checking patients condition and preparing for patient’s discharge


                 Read the excerpt dialogue below between patient and nurse.
                 Fill in the available patients discharge form based on the information from the dialogue.


                Nurse Amina         :  “Assalamu alaikum, Mr. Ahmad. Today, we’ll go over your discharge plan. I
                                      need to ask a few questions to make sure you have the necessary support at
                                      home.”
                Mr. Ahmad           :  “Wa alaikum assalam, Nurse Amina. Sure, go ahead.”
                Nurse Amina         :  “First, when you’re discharged, will you be living alone or with someone else?”
                Mr. Ahmad           :  “I’ll be living with my daughter. She’ll be taking care of me.”
                Nurse Amina         :  “Great. I’ll note that. Now, where will your room be located at home?”
                Mr. Ahmad           :  “My room is on the first floor, so I won’t have to climb stairs.”
                Nurse Amina         :  “That’s convenient. Could you describe the lighting in your room? Is it well-
                                      lit?”
                Mr. Ahmad           :  “Yes, it’s quite adequate, especially during the day.”
                Nurse Amina         :  “Excellent. And how far is the bathroom from your room?”
                Mr. Ahmad           :  “It’s very close, just a few steps away, about three meters.”
                Nurse Amina         :  “I’ll mark that down. Will you have a seated toilet or a squat toilet?”
                Mr. Ahmad           :  “A seated toilet.”
                Nurse Amina         :  “Good to know. Now, let’s discuss any equipment or assistance you may need.
                                      Are there any basic needs where you’d need support?”
                Mr. Ahmad           :  “I may need some help with bathing initially, but I think I can manage most
                                      other things.”
                Nurse Amina         :  “Okay, noted. Do you need any medical equipment at home, like oxygen or a
                                      walker?”
                Mr. Ahmad           :  “No, just some medications. I don’t need any special equipment.”
                Nurse Amina         :  “Understood. Finally, is there any additional assistance or special care you’ll
                                      require once you’re home?”
                Mr. Ahmad           :  “No, just some help with meals and general supervision from my daughter.”
                Nurse Amina         :  “Alhamdulillaahirobbil'alamiin. That’s good to know. Thank you for answering
                                      these questions, Mr. Ahmad. I’ll make sure all this information is in your
                                      discharge plan.”
                (After completing   :  “Allahumma rabban naas mudzhibal ba'si isyfi antasy-syaafii laa syafiya illaa
                the paperwork)        anta syifaa’an laa yughaadiru saqoman.”
                Mr. Ahmad           :  “Ameen. Thank you, Nurse Amina. I appreciate your help.”
                Nurse Amina         :  “You’re very welcome. Assalamu alaikum, Mr. Ahmad.”
                Mr. Ahmad           :  “Wa alaikum assalam.”
