Confirming ”Right Dose” "Your dose today is [amount]. Does that match what’s in your treatment
plan?"
"I’m giving you [amount]. Does that sound correct based on what you were
told?"
"Just to confirm, the prescribed dose is [amount]. Does that look right to
you?"
Confirming ”Right Route” "I’ll be giving this to you [route]. Is that how you usually receive it?"
"The doctor ordered this to be administered [route]. Is that how you’re used
to it?"
"This goes [route], right?"
Confirming ”Right Time” "I have this scheduled for [time]. Does that fit with your regular timing?"
"It’s time for your [medication name]. Is now the usual time for you?"
"You’re scheduled to take this now, at [time]. Does that work for you?"
Asking about Previous "Are you currently taking any other medications, or have you taken any
Medication and Allergy recently?"
"Have you been on any other medications for your condition recently?"
"Are you aware of any allergies, particularly to [medication or substance]?"
"Could you tell me if you’re allergic to any drugs, foods, or other
substances?"
Informing Possible Side "After taking this, you might feel [mention side effects]. Call me if you notice
Effect(s) anything unusual."
"Be aware that some people feel [side effect, e.g., dizziness] after taking
this. I’ll check back on you soon."
"This medication can sometimes cause [side effect]. Do you have any
questions about what to expect?"
"Just so you know, you might notice [side effect]. Please let us know if you
feel any discomfort."
