4. Potential adverse reactions that could result from not taking the medications as prescribed.

   Staff can access this information online through Practice Fusion, at drugs.com, by contacting the
   Executive Director, Medical Director or CEO.

Training

   1. All staff members will be trained to assist a resident in the self-administration of medications
       and will have access to resident medications.

   2. Training is provided by a personnel member authorized to train on behalf of the agency that
       has been trained by a medical practitioner or a registered nurse.

   3. Training includes:
       a. Knowledge of medications commonly prescribed for behavioral health issues and their
            common benefits, side effects and adverse reactions.
       b. Knowledge of the signs, symptoms, or circumstances indicating that a resident should not
            take a medication and of who to contact to review and address the resident's situation
       c. Knowledge of the differences between assisting in the self-administration of medication
            and medication administration
       d. Knowledge of assisting in the self-administration of medication and the medical
            terminology used
       e. Knowledge of the signs, symptoms, and indicators of toxicity or overdose and skill in
            identifying those
       f. Identification of medication errors and medical emergencies related to medication that
            require emergency medical intervention;
       g. The process for notifying the appropriate entities when an emergency medical intervention
            is needed; and
       h. How to document on the MAR forms.

   4. Training skills must be demonstrated. Demonstration of skills must be documented in the
       personnel record.

   5. Training is updated once every 12 months and documented in the personnel record.

   6. A trained staff member must always be present at the facility when a resident who needs
       assistance in the self-administration of medication is present at the facility.

   7. Staff members are responsible to ensure training is up to date.

   8. A yearly review of the personnel records, according to the Personal Review policy and
       procedure, will determine whether a staff member needs training. Staff members may be
       placed on suspension until training is obtained.
